REVISED-Open Enrollment – Health Insurance Rates – Final health plan renewal rates were approved at the March 18th IPBC Board Meeting.  The PPO reflects an increase of 3.9%, while the HMO premium will decrease -7.9%.  The new dental rate (through Delta Dental) decreased -5.4%.  Basic life insurance (provided by FPWC at no cost to employees) increased from $0.092 to $0.100 per $1,000 benefit (from $2.24/per person per month to $2.40).

Open enrollment for coverage effective July 1st will begin Wednesday, May 5th and will end at close of business on Wednesday, May 12th, 2021.  No action is required from you unless you are changing plans.

Dependent Audit – Employees who have a spouse and/or children included with their medical, dental and/or vision coverage will soon receive a notice regarding the need for verification of dependents.  Stay tuned!

Dental Plan Change – MetLife to Delta Dental – Last fall the IPBC conducted an RFP for the dental plan, and on January 28, 2021, the IPBC Board of Directors approved a change from the MetLife Dental plan to Delta Dental effective July 1, 2021. Details will be forthcoming.

Delta Dental of IL-Provider Listing – effective 7/01/2021

Form 1095-B (For those covered under the United HealthCare PPO only)The Affordable Care Act (ACA) requires most Americans to maintain minimum essential coverage.  Because the individual shared responsibility payment is zero in 2020, employees will not need the information on Form 1095-B to compute their federal tax liability or file an income tax return with the IRS.  Nonetheless, FPWC is required to furnish Form 1095-B to individuals covered under the UHC PPO upon request.  Instead of automatically receiving this form as you have in years past, employees must request the form be provided to them by either:

    • Emailing Kim O’Malley, Human Resources Manager at;
    • Calling FPWC Headquarters (815) 877-6100;
    • Visiting Headquarters in person at 5500 Northrock Drive, Rockford, IL  61103.
    • Employees will be furnished a 2020 Form 1095-B within thirty (30) days of your request being received.  Contact Kim in HR with any questions.
